The
American Diabetes Association (ADA) has awarded the Education
Recognition Certificate to Saint Anthony Hospital’s Center for Diabetes, Nutrition, Obesity and Metabolism on behalf of our quality diabetes self management education.
Our program, under the direction of Patrick Conlon, MSN, NP, CDE,
was the first
program in the nation to be recognized under the new criteria for
diabetes education self-management.
Effective education about the
management of diabetes can prevent some of acute complications of
diabetes and thus prevent unnecessary hospital admissions.
The Center offers support for all
patients, and through increased knowledge and awareness of diabetes,
patients can and often do assume a major part of the responsibility for
diabetes management.
Saint Anthony is able to offer our patients
blood sugar meters and other tools to better manage their diabetes.
There are currently 19 recognized education programs in the City of
Chicago, and only 87 in the entire State of Illinois.
Our diabetes program at Saint Anthony provides state of the art information
and education to all individuals with diabetes while helping patients
understand and control their disease. The Center works with the primary
care doctor, patients and their families to provide quality diabetes
care and education to help patients live a healthy life with diabetes.
The Center also coordinates specialty care such as eye, foot and wound
examinations.
Services are provided in English and Spanish.
