MEDICAL RECORDS

To obtain medical records for Saint Anthony Hospital patients:

Print and complete the Release of Medical Records Authorization Form (PDF: 1 page / 46 KB) or request a form by contacting the Health Information Department at the location below. When you complete the form, be sure to specify the dates of service and the type of information needed.

Please note there is a charge for copies of medical records.

To determine size of your records and approximate cost, you may call Saint Anthony Hospital Health Information Management department at (773) 484-1000.

Records may be mailed to the patient or may be picked up in person after a Release of Medical Records Authorization form has been received and processed. The Health Information Management department will notify you when records are available to pick up. Those picking up records will be asked to show photo identification.
